DOWN LOAD The Youth Quests Application Fill the application out by typing into the fillable document and printing it out, or printing it out and fill in the blanks clearly

a. Please download the PDF file. Please fill out the form by typing the information into the form and then printing it out. Have your pastor fill out his portion of the form and e-mail, or mail to us at Youth Quests along with your registration fee.

b. There is a $32.00 Registration Fee due at the time of signing up for the team.

Letters of support will be sent to you after your registration is processed.

Once your application has been completed, reviewed, and processed, you will be placed on your Youth Quests Team. Shortly you will receive your TEAM PACKET including your Youth Quests Team Manual, fund raising materials and ideas. If you have any questions please Contact Us.
