5. COMPLETELY FILL IN YOUR TRAVEL RELEASE FORMS AND SEND IN
In order to participate on a mission trip with Youth Quests, you must have a completed Travel Release Form to the right on file. (If the form does not download, right click and Save Target As or e-mail us requesting it.)
UPDATE YOUR IMMUNIZATIONS Make sure your immunizations are up-to-date. This includes all childhood immunizations and tetanus within the last 10 years. Check with your local family doctor or school for more information. All country-specific immunization recommendations will be referred to the Center for Disease Control on the web at www.cdc.gov.
SECURE INTERNATIONAL HEALTH INSURANCE If you have insurance that covers you internationally, make sure to include that information on your Travel Release Form. If you are unsure, contact your insurance company. If you do not have international coverage, learn about International Health Insurance by visiting the International Health Insurance site. You may purchase international coverage through this company or any similar company. Include a copy of your insurance card or other proof of your international coverage when sending in your Travel Release Form.
TRAVEL RELEASE FORMS
Parents: Sign and notarize the Travel Release Forms if team member is under the age of 18, both parents' / guardians' signatures are required. The parents / guardians and missionary must sign this form in the presence of a Notary Public. Without signatures signed in the presence of a Notary Public, the Travel Release Form will not be accepted.
BACKGROUND CHECK FORM
If you will be 18 years old or older by the day your trip starts, Youth Quests policy requires that you have a valid background check performed each year, which is done when you fill out and send in the Background Check Form. An additional $25 processing fee will be applied to your missions account for your background check.
*** If we do not receive your background check form by your 'ticketing deadline,' you will be charged an additional $75 late fee.
